At your first visit, we will ask for current insurance information. We may ask you to confirm this information at subsequent visits so we can remain up-to-date and fully informed to serve you. Insured patients will receive cost estimates broken down by insured and uninsured costs, and we ask that uninsured costs be paid the day services are rendered. As a courtesy to you, we file all claim forms electronically, provide postage for special claims, and track claims for you.
For patients covered by insurance, we will accept assignment of benefits. This means that you sign the portion of your insurance that “assigns” payment to our office. Most dental insurance plans do not cover 100% of treatment costs. It is because of this, and the extreme delay in receiving payment from insurance companies, that you are asked to pay your deductible and your portion of charges when services are provided. Please note that estimates are based on information provided to us by your insurance and are not a guarantee of payment. Only after a claim is submitted and reviewed by your insurance company can final payment be determined.
We accept cash, checks, debit and most major credit cards (MasterCard, Visa, Discover, and American Express). We also accept Flexible Spending Account (FSA) monies and Health Savings Account (HSA) monies. Additionally, we offer flexible financing options (see below) because we understand that monthly payments can help patients and their families fit the cost of dental treatment into their budgets.
We offer financing through CareCredit® for those who qualify. With CareCredit, you can finance 100% of your dental treatment with no upfront costs, no pre-payment penalties, and no annual fees. Apply online now from Dental Solutions’ CareCredit listing.
Please download our new patient forms, print them out, and bring the completed forms to our office to expedite the paperwork at your first visit for your convenience. Adobe Acrobat Reader is needed to download these forms.
We schedule one patient per appointment so you can get the exclusive, personal time with our doctors and staff that you deserve. We strive to run on time because we understand that you are busy and your time is valuable, only deviating from our schedule in the event of a dental emergency that needs to be taken care of immediately. We ask you to arrive for your appointments on time as well.
Please call at least two business days in advance for changed appointments. Missed appointments without this notification, or repeated cancellations, will incur cancellation fees. We want to work with you to schedule convenient appointment times for your visits to our office.
To help you remember scheduled appointments, we use DemandForce®. This system enables us to use emails and text messages to your cell phone to conveniently remind you of your appointment weeks, days, or even hours before you are scheduled to visit us.
